Create a form
Forms in ApexMentum let you collect structured information from patients — intake history, symptom screeners, consent signatures, follow-up surveys. Patients fill them out from the portal; submissions land in their record.
Open the form builder
From ⚙️ Clinic Setup, click Forms. You'll see your existing forms and a + New form button.
Build a form
Each form has a name, a short description, and a list of questions. Questions support the following types:
- Short text — name, occupation, etc.
- Long text — open-ended history, symptom descriptions
- Yes/no — quick screening questions
- Multiple choice — single-select from a list
- Checkboxes — multi-select
- Number — height, weight, age (with optional units)
- Date — DOB, symptom onset
- Scale (1–10) — pain, energy, sleep quality
- Signature — for consent forms
- File upload — when you want outside lab results or photos attached
For each question, you can set:
- Required vs. optional
- Help text shown below the question
- Conditional logic (show this question only if a previous answer is X)
Templates to start from
When creating a new form, you can start from a blank canvas or one of our templates:
- Initial intake — demographics, history, current concerns
- Functional medicine intake — extended diet, lifestyle, stress, sleep
- Pre-visit symptom check — short list of "what's bothering you today"
- Consent — telehealth
- Follow-up survey
Templates are a starting point — you can edit anything before saving.
Save and assign
Once you're happy, click Save. The form is now available to:
- Assign to a specific patient (see Assign a form to a patient)
- Add to your portal's required intake (every new patient fills it on signup)
- Embed in your public website's booking flow
Multi-page forms
For longer intakes, split your questions into sections. Each section becomes its own page in the patient's view, with a progress bar. This dramatically improves completion rates compared to one massive scroll.
Versioning
When you edit a form that's been submitted before, ApexMentum keeps the old submissions tied to the version they were filled out under. The new version is used going forward. You don't have to maintain "v2 — DO NOT USE" copies.
