Review form submissions
When a patient submits a form, you get a notification and the submission appears in their record.
See what's new
There are three places to find pending submissions:
- 🔔 Notifications in the top-right shows recent submissions across all patients.
- Dashboard has a "Pending review" card listing forms waiting on you.
- Each patient's Timeline shows a 📋 entry for every form they've submitted.
Read a submission
Click into any submission to see:
- The form name and the version that was filled out
- The patient's answers, question-by-question
- The submission timestamp
- Time spent filling the form (helpful for spotting rushed responses)
Long-form text answers are shown in full. Signatures appear inline as the patient drew them.
Mark as reviewed
After you've read through, click Mark as reviewed at the top. This:
- Removes the submission from your pending list
- Logs that you reviewed it (in the audit log)
- Stamps it with the date so anyone else looking knows it's been seen
Add a private note
The note field on a submission is staff-only — patients never see it. Use it for things like "Mentioned X in intake — need to follow up at next visit."
Pull answers into a chart note
For any answer, click the ⋯ menu and choose Copy to chart note. This drops the question and answer into the patient's current chart note, formatted cleanly. Useful for pulling out "Current medications" or "Allergies" from an intake.
Pin important answers
Some answers belong at the top of the patient's record — e.g. allergies, current medications, emergency contact. Click Pin on those answers and they show up in the patient header on every page.
Exporting
To export a single submission as a PDF (e.g. for an outside referral or for a records request), click Export → PDF on the submission view.
For bulk export of all submissions on a form, open Clinic Setup → Forms → [form name] → Submissions → Export all. You'll get a CSV with one row per submission.
Searching across submissions
The Forms search (in the global search bar at the top) lets you find submissions by answer content. Useful for compliance ("show me everyone who answered yes to family history of diabetes").
